New Customer Request

Thank you for your interest in becoming an authorized High Five Sportswear dealer. All new dealer requests are subject to review by the appropriate sales personnel and can take as long as 24-hours. Prior to approval, High Five must obtain/verify:

• Multi-State Tax Exemption Form. The following states require the forms listed in addition to the Muli-State Tax Exemption Form:

• Potential dealer is one of the following businesses: team dealer, sport specialty (i.e. soccer specialty, volleyball specialty, baseball/softball specialty etc... with more than 75% of total sales in one specific sport category), sporting goods, decorator, or promotional products focused on the team uniform market
• Not associated with a school, league, recreational organization, or are a team player or coach
